Background

The main objective of the project is to improve social integration and rehabilitation of the children with cardiac and other chronic diseases through providing of the unify complex social services

Specific objectives: 1. Teamwork for unifies the proposed social practice to the target group 2. To guarantee full access for the families and children to all services in the three regions — Sofia, Bourgas, Dobrich 3. Realization of health rehabilitation programs for children and teenagers with chronically diseases and their health coeval 4. To assure safe space for training skills, behavior and communication 5. Increasing the information and attention of the medias and society through problems and abilities of the chronically diseased children.

Activities: Medical, psycho-social, educational counseling for children and their parents, club activity for children, teenagers and parents, training in different skills, health care and rehabilitation programs and services, public activities — brochures, informational bulletin, media events

Practice
Results

Built up and functioning multidisciplinary team from the members of the three organizations; Unified complex social services for children with chronic disease and their families. Effective and adequate model for coordination between different specialists and services, who work with the target group; easiest access to the proposed social services; Increased capacity and skills of the children and their parents to deal in difficult situations, for fast and adequate decision making in crisis, improved of their resources for participating in the society and coming out from the social isolation; Improved social integration for the target group; Financial help for 82 families, who underwent their check up in their towns from leading specialists from the leading organization, as well as for 159 families, who used the social bedrooms during their stay in Sofia; over 720 families benefit from the proposed services on the project; over 92 children and teenagers participated in the club activity; participation of 120 children and youngsters in the 5 health care programs; Improved community attitudes towards people with chronic diseases; 1050 pieces of informational bulletin and 1500 brochures.
